Cone-beam computed tomography (CBCT) is a modern X-ray method that creates a three-dimensional digital image of the jaw and teeth. Where an ordinary panoramic X-ray shows the jaw in only two dimensions, flat, a CBCT reveals the height, width and density of the bone and the exact location of anatomical structures in three dimensions. That is why 3D dental tomography has become the key diagnostic tool of modern implant planning. What is a cone beam CT (CBCT)?
CBCT stands for cone-beam computed tomography. In a single rotation around the head the machine collects many images using a cone-shaped X-ray beam; a computer combines them to build a three-dimensional model of the jaw, teeth and surrounding tissues. As a result the dentist can view structures in any plane — slice by slice — and assess the bone from every side. Unlike conventional (medical) computed tomography, dental CBCT is designed specifically for the maxillofacial region and usually works with a lower radiation dose.
How CBCT differs from a panoramic X-ray
A panoramic X-ray (orthopantomogram) is a useful and widely used method, but it shows a three-dimensional jaw on a single flat image. On this two-dimensional image structures can overlap, and the width and depth of the bone are not precisely measured. CBCT reveals the same area in three dimensions: the dentist sees the bone volume in real measurements and pinpoints the exact location of the nerve canal and the maxillary sinus. In many simple cases a panoramic X-ray may be enough; but for implant planning, complex anatomy or a likely need for bone augmentation, the three-dimensional data from a CBCT is often decisive. Which examination is needed is decided by the dentist based on the clinical situation.
Why is CBCT important for implant planning?
The success of an implant depends greatly on placing it in the correct position — within enough bone and at a safe distance from important anatomical structures. CBCT makes it possible to take these decisions before surgery:
- Measure the bone volume and assess its density — determine whether there is enough for an implant.
- See the exact location of the lower-jaw nerve, the maxillary sinus and other structures, and keep a safe distance from them.
- Where needed, plan bone augmentation (for example, sinus lifting or grafting) in advance.
- Determine the implant's optimal size, angle and depth in advance in digital planning software.
Radiation and safety
CBCT carries a low dose of ionizing radiation. The dose of a dental CBCT is usually considerably lower than a conventional (medical) CT of the same area, and somewhat higher than a single panoramic X-ray; the exact dose depends on the machine, the size of the field of view and the settings. The key principle for radiation is ALARA — "as low as reasonably achievable." So a CBCT is ordered only when clinically necessary, the field of view is limited to the area needed, and protective measures may be used. The diagnostic benefit usually outweighs the low-dose risk, but the dentist makes this decision based on an individual assessment. Be sure to tell your dentist about pregnancy or other special conditions.
How the scan works and what to expect
A CBCT is a fast, painless examination that needs no special preparation. Here is briefly what to expect:
- Before the scan you may be asked to remove metal objects (earrings, a removable denture, hair clips), as they interfere with the image.
- You sit inside the machine or stand; your head is held still by a soft support.
- The machine slowly rotates once around your head; it is important to stay still during this. Radiation occurs only during this short rotation.
- The scan usually takes only a short time — often under a minute — while the actual radiation exposure is just a few seconds; the computer then builds the three-dimensional model, and the dentist uses it for planning.
